Proof of Service Forms
Complete the Proof of Service form to before serving the petition on the affected party(ies). This form notifies them that you are submitting a request to the City of Mountain View for an adjustment in rent.
Representative Authorization Forms
Complete the Representative Authorization Form to designate a representative to file this petition, make decisions, or appear on your behalf during the petition process. Once completely filled out and signed, submit to the Rent Stabilization Division.
Response Forms
When a petition is filed, the affected party(ies) can respond to the petition by completing a petition response form and submitting it to the Rent Stabilization Division. If a Tenant Hardship Petition is filed, landlords should respond using the Tenant Hardship Petition Response Form.
